ABOUT LLCHC -Lower Lights Christian Health Center (LLCHC) transforms the overall health of Central Ohio, serving one individual at a time. We are focused on whole-person wellness, available to ALL in Central Ohio who need it, regardless of ability to pay! - -In 2019 alone, we served over 12,000 patients - with 40% being uninsured - and totaled 50,000+ medical encounters!Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral health care, 340B pharmacy, nutritional assistance programs, and more. -Working hours are Monday - Friday -with occasional Saturday morning coverage.The -Care Manager is responsible for managing and coaching patients with selected illnesses with a goal -to improve their health outcomes.QUALIFICATIONS:

• Medical Training and/or Certifications RN (*licensed in the State of Ohio - REQUIRED)
  • One to three years full-time experience in a medical practice.
    • Ability to work as a team member.
      • Computer experience desirable.
        • Ability to work with a culturally diverse group of people.
          • Must be willing to work flexible hours as required by position.ESSENTIAL JOB RESPONSIBILITIES: -Contact with patients in order to assist them in improving their health outcomes.Maintains a specified regimen of contact with specific groups of -patients identified by their PCP and assigned for intensive care management.Educates the patient on self-management of their illness(es)Coaches the patient in developing attainable goals for improvement of their -conditions.Consults with the patients PCP when there are questions about or changes in the patients progress.Documents relevant Care Management information in the patients chart.Identify barriers to improvement of the patients condition(s).Adapt the goals and plan for progress, if needed.Alerts the PCP if there is any information that comes out of the care management activities that could endanger the patients health or is contrary to clinical guidelines. -Data Management and tracking with patients in the intensive care management program. Track progress on patients and report to appropriate parties at required intervals.Prepare reports with program results for review in CQI -Quality Department SupportCQI (Quality) meeting participation and preparation of necessary reportsSupport Quality Department with other projects as neededBENEFITS AND PERKS
