Public/Community Health opening in Columbus, Ohio. TITLE: POPULATION HEALTH CARE MANAGEMENT MANAGER
LOCATION: Columbus, Ohio Are you considering a career change?
Looking for a smaller community(25K population), that offers a low cost of living(18% lower than US average) and plenty of activities in the area but is within an hour of a large city.
If so, an opportunity to work with an integrated health care system based in east central Ohio is looking for someone to join their team.
The system is a not-for-profit and has over 300 physicians and multiple outpatient care centers throughout the area.
The system is the largest provider in a six county region and a primary referral center.
It offers a higher level of service than what is typically found in a community of its size including open-heart surgery, trauma care, a Level II neonatal intermediate care unit, neurosurgery and
comprehensive cancer services.

This position drives department performance in key care management areas which include excellent care coordination between all areas of System Wide Care Management including inpatient, transitional, disease based, procedure based and longitudinal care management.
The incumbent oversees process standardization for SWCM to follow ensuring communication between care team members so that patients are moved through the continuum of care in a safe, efficient and cost- effective manner.
Also works to build community collaboration for safe, cost effective, efficient care transitions.
Uses expertise to actively engage in coordination of care with internal and external stakeholders, community resources and the Health Care System.
-Analyzes department related information regarding operations, budgets, staffing reports, and/or other activities, and develops action plans based on identified trends and variances.
-Creates standard work for all System Wide Care Managers.
-Establishes KPIs, effectively communicates and monitors KPIs in alignment with the System initiatives and scorecard metrics and objectives.
-Works collaboratively with leaders in the organization in a dyad operating model to assure success with the team.
-Demonstrates understanding of operational issues in areas of responsibility providing support as needed for staff.
-Participates in interdisciplinary development of evidence-based patient standards of care and the collection and analysis of patient outcomes. Includes facilitating participation of staff in same.
-Develops, implements, and evaluates systems and processes that complement the overall system for performance improvement: uses data to drive decisions, plan and implement performance improvement strategies for case management.
-Actively represents Care Management on hospital committees and teams and facilitates opportunities for employees to do the same.
-Demonstrates effective human resources management. Responsible for appropriate recruitment, selection, and retention of staff, attempting to minimize turnover of productive employees.
-Delegates effectively by establishing clear expectations, knowledge to do the job with appropriate follow-up mechanisms while maintaining accountability.
-Handles escalated cases when needed; builds strong relationships to support staff communication with same.
-Develops relationships with preferred providers for skilled nursing, home health, LTACH and other community based care sites. Develops and maintains liaison programs with each as appropriate.
-Prepares or coordinates the preparation of periodic and special reports required by the Healthcare System.
-Actively engages with other departments of the Healthcare System on issues such as care coordination, contracting, billing, reimbursement and denials. Collaboratively initiates improvements related to these areas and engages with same group to maintain hospital and CIN compliance with the Quality Improvement Organization (QIO) and CMS guidelines.
-Effectively prepares reports and correspondence within specified time frames involving synthesizing information, writing text, and preparing final communications.
BSN or Bachelor s degree in health related field with current RN license in the state of Ohio, or Master of Social Work/Counseling with current license in the state of Ohio.
Three (3) years of work experience in a hospital or inpatient care setting.
Three (3) years of management or demonstrated leadership experience required.
Reports to Director of Population Health Operations.
This is a new position due to growth of the healthcare system.
Position will offer between $33.60hr-$46.24hr depending on experience and comes with a full benefit package and relocation.
