Be a part of the team leading the nation in healthcare.4432 - ProMedica Home Health Care - Marion, OH - Serving Central Ohio Location4432 - ProMedica Home Health Care - Marion, OH - Serving Central OhioEducational RequirementsCurrently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.Position Requirements6 months Experience requiredDaily TravelJob Specific Details:Administers nursing care according to the patient?s plan of care plan and needsCare for multiple patients throughout the day in their individual home environmentsTeach the patient and/or family members about new diagnosis, care, and support skillsCoordinates and delivers nursing care to patients and familiesAssesses & evaluates patient?s level of care, needs and requirementsAssesses the patient?s response to the Plan of Care and recommends changes as neededParticipates in on-call coverageProvides education to the patient/family regarding the disease process, self care techniques, preventionstrategies, rehab nursing procedures and community resources for discharge planningInitiates appropriate preventive and rehabilitative nursing proceduresPrepares clinical and progress notes that demonstrate progress toward established goalsInforms the physician and other personnel of changes in the patient?s needs and outcomes of interventionImplements the individualized plan of care and recommends revisions to the plan as necessary.
