Job Description:
Seeking a Performance Improvement Manager Home Health RN to
coordinate development, monitoring, and management of Performance
Improvement Program (PIP) for offices.
As a Performance Improvement Manager Home Health RN, you will be
responsible for:
- Coordinating the development, monitoring, and management of the
Performance Improvement Program (PIP) for offices
- Advising offices on Performance Improvement Plans
- Providing education and guidance to offices on clinical,
compliance, and performance improvement issues
- Assisting offices to achieve performance improvement objectives
utilizing benchmarking data

- Maintenance of open communication with administrators/managers
and clinical leaders of offices
- Collaboration with Corporate Clinical Services Team to identify
company-wide areas for improvement and educational needs
- Reporting of concerns/audit results/office visit findings to
appropriate personnel
- Communication of weekly schedule, project work, office specific
plans, upcoming audits, and office visits
- Quality, Compliance, and Clinical Operations:
- Advising assigned offices on Performance Improvement
projects
- Review and submission of plans to Governing Body
- Ensuring plans are followed and quarterly updates
occur
- Providing oversight of the QAPI program for assigned
offices
- Collection and analysis of outcome data to develop education
and guide assigned office's Quality Assurance & Performance
Improvement (QAPI) initiatives
- Support and guidance of ongoing performance improvement efforts
of offices to improve star ratings for patient outcomes of care and
patient experience
- Performance of clinical records audits based on identified need
and/or by individual performance improvement initiatives of
offices
- Employee Retention:
- Participation in onboarding program for new clinical
managers
- Utilization of a collaborative approach when interacting with
office staff
- Encouragement of open dialogue regarding agency issues to
improve overall compliance with expected standards
Requirements:
- A Registered Nurse with a license in good standing in at least
one state in the assigned region
- Three (3) years of experience as a nurse and one (1) year of
experience in home health
- One (1) year of experience with and/or knowledge of performance
improvement activities
- Good knowledge of home health Medicare Conditions of
Participation and state licensure regulations
