Learn more at ! - The Hospice Chaplain provides for the spiritual and emotional needs of patients, visitors and staff, respectful of their cultural, spiritual and religious beliefs, within the framework of a professional, progressive model of chaplaincy. Requirements - Education: Master's degree required in Divinity, Theology, Pastoral Ministry, Pastoral Counseling or, equivalent education required by the applicable certifying body. - Licensure / Certification: Four units of accredited Clinical Pastoral Education completed from an accredited ACPE center. Certification as a chaplain in the National Association of Catholic Chaplains (NACC), Association of Professional Chaplains (APC); or National Association of Jewish Chaplains (NAJC) required; or, eligible to be certified. Must be certified by one of the above certifying bodies within three years of employment if not currently certified. Expected to maintain certification once received. - Current knowledge of the theological, medical-moral issues which supports the teaching of the Roman Catholic Church (i.e. Ethical and Religious Directives of Catholic Healthcare Facilities) and supports the mission of MCHS. - A basic understanding of health care organizations and procedures. - Ability to work autonomously and in a variety of team environments. - Proficient at multi-tasking and having a high level of problem solving skills. - Demonstrates resiliency with matters of death and dying. - Maintains patient confidentiality according to MCHS policy. - Criminal background check and fingerprinting completed as required by Ohio law Responsibilities - Serves as a resource for the spiritual/emotional needs of patients, visitors, and MCHS associates and develops positive relations exhibiting respect, responsibility and cooperation at all times. - Shares in the responsibility of providing 24-hour pastoral care coverage through duty, back up chaplain rotation and weekend call. - Assists patients and their loved ones with ethical decision-making assistance (e.g., end of life decisions, organ-tissue donation decisions, and information and execution of advance directives). - Conducts spiritual assessments of patients and families and records appropriate information in the patient's medical record according to the Spiritual Care Services Documentation policy. Participates in continuity of patient care through reporting, logging and communication with appropriate staff - Provides spiritual care to Hospice patients/families at home or in-patient settings according to the Standards for Excellence in Spiritual Care established by the Trinity Health Care System, regulatory agencies and national competency standards for Hospice chaplains. - Responds according to departmental procedures on all emergency and routine referrals for spiritual care services. - Completes spiritual care according to national benchmark productivity standards. - Assists patients and their loved ones with ethical decision making concerning advance directives of the State of Ohio. - Assesses/reassesses spiritual needs of patient/family through Interdisciplinary Team (IDT) collaboration and during each visit or contact and consultaton with clinical manager as needed. - Develops spiritual plan of care, implementing a wholistic, patient-focused approach and documents plan after each visit. - Fosters a work culture that promotes collaboration, connection, diversity and positive regard for patients/families, colleagues, volunteers and leadership. - Serves as a resource for the spiritual/emotional/cultural needs of patients, visitors, volunteers and associates. - Plans and participates in the provision of ecumenical remembrance services., the annual homecoming and Reclaiming Special Days and Seasons programs. - Acts as a mentor to new associates, students in social work, pastoral care (CPE students), nursing, and also to volunteers. - Attends necessary and mandatory in-services required of Spiritual Care Services Associate Chaplains, this includes: annual MCHS education, designated requestor training for the organ/tissue/eye procurement agencies with which Mount Carmel has contracts and any site and/or system staff meetings as requested. Demonstrates a willingness to improve their skills as expressed in the MCHS competencies for Spiritual Care services. - Provides grief support as needed (e.g.,presiding at funerals and memorial services).
