Description :
As a member of the Community Hospice Disciplinary team, the nurse works under the general direction of the medical director and or attending physician in the provisions of skilled nursing assessments, planning and coordination of services. -This is done in order to maximize the comfort and health of patients and families. -Participates in contracting facilities and community member, as well as meeting physicians and groups to market hospice services.

Education & Qualifications :
- - - - - - - - -Graduate of an approved School of Nursing accredited by the National League of Nursing. A Baccalaureate degree is preferred.
- - - - - - - - -Current license to practice nursing in the State of Ohio.
- - - - - - - - -Must be CPR/First Aid Certification
- - - - - - - - -Ohio driver's license, current auto insurance and access to a car during working hours.
- - - - - - - - -Home Health experience with terminally ill patients preferred.
- - - - - - - - -Demonstrate flexibility, excellent assessment skills, and ability to supervise others and the confidence to work independently.
- - - - - - - - -Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.

Duties :
- - - - - - - - -Identifying the patient's and family's physical, psychosocial, environmental, safety and developmental needs and individualizing the patient's plan of care based on the needs identified. -Assume responsibility for a patient/family caseload that includes the assessing, planning, implementation and evaluation of the nursing process.
- - - - - - - - -Assessing all aspects of the patient's pain and developing an individualized pain management plan; anticipating, preventing and treating undesirable symptoms or secondary symptoms.
- - - - - - - - -Identifying and addressing comfort care needs. -Supporting, instructing and educating the patient, family and caregiver.
- - - - - - - - -Documenting problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes achieved from intervention and care provided.
- - - - - - - - -Coordinating all patient and family services.
- - - - - - - - -Maintaining the dignity of the dying patient, supporting the patient's and family's unique spiritual and cultural beliefs.
- - - - - - - - -Providing holistic, family-centered care across treatment settings to improve the quality of life; and consulting and collaborating with the interdisciplinary team and others involved in the patient's core.
- - - - - - - - -Initiate communications with attending physicians, other hospice team members and agencies as needed to coordinate optimal care and use of resources for the patient/family.
- - - - - - - - -Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and communicate that data in team meetings to formulate an Interdisciplinary Plan of Care.
- - - - - - - - -Attend interdisciplinary team meetings to facilitate discussion of patient/family status in order to evaluate and revise the interdisciplinary team plan of care.
- - - - - - - - -Supervise and document the home health aide plan of care every 2 weeks.
- - - - - - - - -Share in providing 24-hour on-call support 7 days a week.
- - - - - - - - -Provide appropriate support at the time of death and period of bereavement.
- - - - - - - - -Participate in orientation and inservice training programs for Jewish Community Hospice staff.
- - - - - - - - -All other duties as assigned

Safety and Sanitation:
- - - - - - - - -Must follow and adhere to all sanitation procedure as prescribed by the Ohio Department of Health and the Columbus Health Department and all other governing agencies followed by WHV
- - - - - - - - -Must assure that WHV safety policy and procedures are followed at all times
- - - - - - - - -Must follow the WHV Infection Control, Universal Precautions, and the Exposure Control Plan, policies and procedures when performing daily tasks and functions
- - - - - - - - -Report all hazardous conditions, accidents, and incidents to the immediate supervisor
- - - - - - - - -Must be able to lift up to 25lbs on a routine biases
- - - - - - - - -Must be able to push/pull up to 50lbs on a routine biases
- - - - - - - - -Must be able to stand for entire shift duration along with lifting, binding, pulling and pushing
