Job Description:
Central Ohio Primary Care Physicians is seeking a Home &
Palliative RN Supervisor for our Comprehensive Care team located in
Westerville, OH. The Home & Palliative RN Supervisor covers a
patient care load in addition to leading the Comprehensive Care
acute care nurses. Together with the manager, the Home & Palliative
RN Supervisor provides guidance on departmental and organizational
policies and procedures, clinical consultation, and problem-solving
to ensure patients receive timely, effective, and high-quality
care. The Acute Care Nursing Supervisor is the subject matter
expert in his/her discipline and will always strive for clinical
excellence, mindful service, and the modernization of primary care.
This position can be a hybrid role upon completion of training. The
Comprehensive Care team will focus on the delivery of care for the
frailest and most clinically complex segments of the population -
the estimated 2-5% of adult seniors that are home-limited/bound and
at highest risk for fragmented and uncoordinated care. The RN will
be part of an interdisciplinary team that will provide Palliative
and Primary Care to patients in their home setting. The RN will
provide clinical oversight to acute care nurses and acute care MAs.
The Acute Care Nursing Supervisor will provide clinical oversight
to acute care nurses and acute care MAs as well as maintain a small
caseload of patients. The Comprehensive Care program aims to keep
patients safe and well at home and to reduce the suffering
associated with advanced illness and/or functional decline. The
team will be experts in establishing patient goals of care and will
implement a care plan to support the patients/ families in
achieving those goals in the home setting. This goal of this role
and team is to favorably impact the following metrics:
- Inpatient utilization
- ED utilization
- 30-day readmission rates
- % of CHPC patients who die on hospice vs without POSITION(S)
SUPERVISED: Acute Care RNs, LPNs and MAs ESSENTIAL FUNCTIONS AND
RESPONSIBILITIES: Responsibilities include but are not limited to:
Leadership and Operational Support --- Support all acute care staff
onboarding and education, time card and PTO approval, staff
reviews, etc. --- Serve as a representative of the Comprehensive
Care program in various settings including internal COPC team
meetings and with external partners (home health, hospice, etc.).
--- Develop and implement workflows that support effective patient
care and positive program outcomes, involving interdisciplinary
team members and internal and external stakeholders as required.
--- Lead and / or represent Acute Care nursing in special projects
that drive care transformation and program effectiveness. Examples
may include: vaccine distribution, home IV administration, remote
patient monitoring, and evening and weekend visit shift creation.
--- Responsible for the ongoing education and professional
development of acute care nursing (RN and LPN) team members.
Facilitation of Patient Centered Care --- Provide assessment,
planning, and direct patient care to patients in accordance with
the plan of care established by the interdisciplinary team. ---
Lead patients and families in discussions and decision-making
regarding advanced care planning, code status, and planning for
future care needs. --- Evaluate, update and revise plan of care to
facilitate achievement of expected outcomes. --- Assess the
educational needs of patients, care givers and family considering
cultural issues and other special challenges. --- Provide teaching
and educational material to the patient and/or caregiver concerning
the following topics when applicable: diagnosis and disease
progression, diagnostic tests and procedures, safe and proper use
of equipment, reason for and safe use of medication food/drug
interactions, injury prevention, self-care, and nutritional needs.
Interdisciplinary Practice --- Participate in the development and
execution of the Plan of Care across the continuum of care. ---
Work in collaboration with internal and external care teams and
community partners as necessary to facilitate continuity of care.
--- Facilitate referrals to other disciplines and internal health
and community based programs as appropriate to improve patient
outcomes. Evidence-Based Care --- Utilize validated Palliative Care
tools such as the PPS, FAST, and ESAS to assess and document
patient status and symptom burden. --- Increases knowledge of best
practices and clinical standards of care and incorporates knowledge
into practice Measurement and Reporting --- Documents in the
electronic medical record in accordance with policies and
guidelines. QUALIFICATIONS: To perform this job successfully, an
individual must be able to perform each essential duty

A. Education, Licensures & Certifications Required: Current Ohio RN
License; BSN preferred Required: Certified in CPR --- Required:
Minimum five years of med/surg, critical care, hospice, or home
health nursing B. Knowledge, Skills & Abilities --- Knowledge of
nursing care principles, procedures, and practices --- Knowledge of
pharmaceuticals, expected beneficial effects, potential side
effects and be skilled in safe administration practices --- Ability
to recognize adverse patient signs and symptoms and to react
appropriately --- Must be able to identify and provide for
age-specific patient needs --- Must be able to assess, interpret,
and appropriately react to key data about a patient's status ---
Demonstrated ability to work independently and within a fast-paced
team environment --- Strong assessment and prioritization skills
--- Ability and comfort in supporting patient care centered around
palliative care goals --- Ability to administer IV/IM medication
--- Ability to perform straight or indwelling catheter procedures,
blood draw, nebulizer treatment, etc. --- Excellent written and
verbal communication skills, especially the ability to communicate
effectively in stressful situations --- Ability to cope well with
change and ambiguity --- Self-disciplined, energetic, passionate,
and innovative --- Must have a high degree of accountability and
comfort in creative problem solving within an uncontrolled setting
(patients' homes)--- Must have reliable transportation and the
ability to travel to patient homes within Central Ohio
