You will collaborate with your team members to assess, plan, implement, monitor, and evaluate the options and services required to meet home health and home care patients needs. You will manage your team from one of our home health and hospice offices.*What it takes** Current Ohio nursing license to practice as a Registered Nurse (RN) required.
