Weekly Pay Range - $2064 To $2293 Job Title: Registered Nurse (RN) Job Specialty: Hospice RN Case Manager Job Duration: 13 weeks Shift: Days - 8 HR: Monday - Friday Guaranteed Hours: 40 hours per week Experience: Must have at least 12 months of hospice in-home settings experience within the past 3 years License: Registered Nurse (RN) Certifications: [Include any specific certifications required or preferred] Must-Have: HCHB experience Job Description: - Provide compassionate end-of-life care and support to patients and their families in a home setting. - Manage and coordinate patient care plans and scheduled visits as assigned. - Ensure compliance with hospice policies, procedures, and regulatory requirements. - Collaborate with interdisciplinary teams to optimize patient care and outcomes. - Maintain accurate and timely documentation of patient care and communication. - Adapt to workload changes, including flexibility for assistance during weekends if needed.
