skills at a level normally acquired through the completion of
bachelor’s degree in human services or behavioral healthcare field,
or Associate’s degree in human services or behavioral healthcare
field with some paid or volunteer experience in the field of
chemical dependency treatment. State of Ohio Chemical Dependency
Counselor Assistant (CDCA) certification required. Outpatient Case

Outpatient Case Manager (CDCA) Job Description Maryhaven, Central

role of Outpatient Case Manager is to work with youth and adults
dealing with behavioral health or substance use concerns by
supporting and connecting them to the necessary services that will
help stabilize their day-to-day life. This role would involve
meeting with individuals in their home, Maryhaven office, and
community. The Outpatient Case Manager will work with our housing
campus patients who have a severe and persistent mental illness and
will also coordinate reentry work with our patients who are leaving
the criminal justice system and returning to Union County. The Case
Manager will complete assessments, help clients to identify
barriers, keep treatment plans up to date, and will be able to
successfully document interactions and sessions within 24-48 hours
of the appointment time. The Case Manager should also have a good
understanding of community resources in and around the Union County
area. The Case Manager will transport clients. Compensation
