Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist – Home Health Location: Marion, OH (home health territory up to a 60-mile radius from the office) Duration: 13 weeks Shift: Day shift, 5x8 hours (08:00 – 17:00) Job Responsibilities and Requirements: The Physical Therapist will provide home health physical therapy services primarily to a geriatric population. Responsibilities include completing start-of-care and follow-up visits, developing and implementing individualized treatment plans, performing evaluations and reassessments, and delivering skilled therapy interventions in patient homes. The therapist is responsible for timely and accurate documentation, effective communication with the interdisciplinary care team, and maintaining productivity expectations of approximately 6–7 patients per day. Travel within the assigned territory is required.
