Job Description:
Mount Carmel Health System invites applications for the position
of Registered Nurse Care Navigator. This vital role offers an
opportunity to join a team committed to high-quality patient care,
with comprehensive services provided across five central Ohio
hospitals. As a Care Navigator, you will focus on coordinating
patient care activities and ensuring an integrated approach to
health services across all facilities, guiding patients through the
complexities of the healthcare system. Develop personalized patient
care plans and coordinate with various healthcare teams. Guide
patients through the healthcare system, from diagnosis through
treatment and beyond. Educate patients and their families on their
condition, care procedures, and resources available. Monitor
patient progress and adjust care plans as needed. Collaborate with
other healthcare professionals to ensure comprehensive service
delivery. Participate in case reviews and consultations within the
healthcare team. Maintain complete and accurate patient records.
Strong interpersonal and communication skills. Exceptional
organizational and case management skills. Ability to multitask and
manage time effectively. In-depth knowledge of healthcare
regulations and policies. Proficient in using medical software for
patient management. Medical Insurance Dental Insurance Vision
Insurance 401k Valid RN license. CPR certified. Certified Care
Manager (CCM) is preferred but not required.
