Job Description:
Monday thorugh Friday with a maximum of 24 hours per week. Are
you an experienced MDS nurse interested in the next step? The MDS
Coordinator provides oversight of the RAI process and conducts
assessments and care plan coordination for guests. - Benefits:

Responsibilities: Completes the MDS, CAA's and care plans within
regulated time frames. Coordinates scheduling the RAI process with
the interdisciplinary team. Assesses resident through physical
assessment, interview and chart review. Discusses resident care
needs with care givers, including physician, nursing, social
services, therapy, dietary, and activity staff. Reviews information
from hospital, consults and outside agencies and uses such
information in the completion of the assessment and care planning.
Coordinates, identifies, and/or initiates significant change MDS.
Is prepared to conduct PPS meetings maintaining MDS assessments per
Medicare schedule and maintains PPS board for monitoring of
Medicare days and RUGs utilization in the absence of the Care
Management Coordinator. Remains current with American Association
of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications Registered Nurse (RN) or Licensed Practical Nurse
(LPN) licensure. AANC certification a plus. RAC-CT. Knowledge of
the Resident Assessment Instrument (RAI) process, including the
principles the Prospective Payment Process (PPS) strongly
preferred. Experience as an MDS Nurse. About Laurel Health Care
