Responsibilities:Complete Substance Use Disorder (SUD)
AssessmentsAssess nature of clients situations by interviewing and
reviewing personal history Develop and execute individual treatment
plansDetermine appropriate milestones to gauge client
progressInteracts professionally and respectfully with
clients.Seeks feedback from clients/co-workers.Abides by and has
knowledge of agency confidentiality policies and HIPPA rules.Takes
appropriate action on complaints/requests of customers.Participates
in Quality Improvement training as needed.Updates and maintains
accurate client documentation.Participates in community and
internal activities.Communicates needed information respectfully
and clearly with clients, providers, and staff.Perform other duties
and special projects as assigned.Updates and maintains accurate
client documentation in accordance with applicable
standards.Demonstrates ability to develop and implement treatment
plan goals and achieve stated outcomes.Identifies need for chemical
dependency and/or mental health treatment.Demonstrates competency
in current treatments, especially those deemed to be Best
Practices.Exhibits ability to de-escalate client crisis situations
in an appropriate therapeutic manner.Has knowledge of and seeks
support of area services and referral sources for clients.Meets
minimum productivity as per agency policy.Assesses personal
training needs, sets goals to meet these needs, and achieves
them.Perform other duties and special projects as
assigned.Qualifications: CDCAPrevious experience in substance use
disorder, social work, counseling, or other related
field.Compassionate and caring demeanorAbility to build rapport
with clientsExcellent written and verbal communication skillsValid
drivers license & insuranceMust have an operating vehiclePay ranges
for the QMHS position are based on experience and level of
licensure. Candidates may select a set salary or variable hourly
wage. Salary QMHS positions are offered a benefits package. This
position requires 95 billable hours per month (est. 25 hours per
week).$38,000 - $41,600 Annual SalaryHourly wage positions are
eligible for benefits with 72 billable hours per month. *Conditions
