Clinically progressive, forward thinking outpatient clinic is recruiting for a full-time ambulatory care Registered Nurse for our innovative hospital-based outpatient Medication Therapies Center. Patient care services offered include anticoagulation, diabetes, COPD, asthma, heart failure, transitions of care, weight loss management, cholesterol management, and medication management (MTM).Ambulatory clinical services focus on educating patients about their disease state and developing individualized disease management plans including medications and lifestyle interventions. Clinical pharmacists and nurse practitioners work collaboratively with referring providers utilizing consult agreements. The Medication Therapies Center includes a team of clinical pharmacists, nurse practitioners, and pharmacy technicians. We are looking to expand our level of care by adding the skillset of a Registered Nurse to support patient triage, our clinical services, and front desk services.This is an excellent opportunity to practice outpatient nursing in a way that utilizes a mix of clinical and administrative skills in the care of patients. We are looking for candidates with experience in ambulatory care and chronic disease state management skills who also have excellent interpersonal communication skills and work well in a team environment.What You'll Do:Utilize the nursing process including assessment, diagnosing, planning, implementation, and evaluation to deliver comprehensive patient care.Direct and coordinate all nursing care for assigned patients, ensuring effective collaboration with interdisciplinary teams.Maintain accurate and timely data collection for direct and indirect patient care.Document patient care activities thoroughly and reflectively on appropriate hospital forms.Educate patients and their families on relevant health information and provide written materials as needed.What You Bring:Graduation from an accredited school of registered nursing with current licensure in the State of Ohio or multi-state with Ohio recognized.A minimum of 2 years of clinical or ambulatory care experienceCompletion of twenty-four (24) contact hours of approved Continuing Education (CE) during each two (2) year licensure renewal period as required by the Ohio Board of Nursing.Continuous certification in American Heart Association's Basic Life Support (BLS).Certification in American Heart Association's Advanced Cardiovascular Life Support (ACLS) within six (6) months of employment and maintenance of bi-annual renewal thereafter.Completion of National Institutes of Health (NIH) Stroke Scale certification within six (6) months of hire and annual renewal thereafter.Completion of Behavioral Health & De-Escalation Training (BHDT) within six (6) months of hire and annual renewal thereafter.Why Join Us:At Memorial, we uphold values of Compassion, Accountability, Respect, Excellence, and Service in everything we do.
